The office accepts all insurance that covers out-of-network Acupuncture treatments. Coverage varies from policy to policy, therefore please answer the questions below and we can figure out if your insurance has acupuncture treatments.

You can also scan or take a picture of your insurance card, front and back, and email it to us. In this case, include your full name and date of birth for you and your policy holder (if applicable). *Until your insurance coverage has been confirmed, all payment is due at the time of service.
